Thank you for choosing Apex Dental! New Patient Forms can be completed prior to your first visit. We encourage you to print it out and bring the completed form with you for your first appointment. If you prefer to complete the form at our office, please plan to arrive early enough to allow yourself plenty of time to fill it out before your appointment time.
